"He smiles inspite of everything"
I think it's wrong?

Well...it's not wrong. Actually, it could be understood that way, however I would say that it's an imperative sentence addressed directly.
To me it means:

"Smile despite everything"

if the sentence translated to the Arabic must have a then pronoun thats ok, although I prefer that it is: "she smiles inspite of everything" , but also "she smiles despite everything" is ok.

¿Puedes aclararnos si la frase es imperativa o no?

¿Es un consejo que está siendo dado?
Si no es, no se puede saber si el "sonrie" se refiere a 'él sonríe" o a 'ella sonríe"
