I think it would sound better this way:
'Please send us your exact mailing address, so we can send you the kit.'
but I stuck with the original wording as we tend to do.
Also, the beginning of the Hebrew original is a bit confused - it seems like they started writing one thing and then changed it without correcting the sentence.
